NDIS services across the Mornington Peninsula

Daily supports, registered nursing, community access, skills and creativity programs. Some are funded through an NDIS plan. Some you can arrange privately. One isn't open yet, and you'll see why below.

What we run

What we deliver, service by service.

Some of these are funded through an NDIS plan. Some you can arrange and pay for privately. We'll go through your plan with you first, and tell you plainly what it covers.

06 Using your plan

Support coordination

Help to turn a plan on paper into supports that actually run. This is the work one of our founders did before J&J, so we know it from the coordinator's side of the desk. If we coordinate your plan and also deliver some of your supports, we'll say so in writing, and you're free to use someone else for either.

  • Working out what your plan funds
  • Finding and connecting providers
  • Coordination for complex or crisis situations
  • Support through hospital and housing transitions

Positive behaviour support

This one isn't open yet. Victoria requires us to appoint an Authorised Program Officer first, and we're working on it. If you need behaviour support now, call us and we'll point you to someone who can help.

  • Not open for referrals yet
  • Authorised Program Officer to be appointed
  • We'll update this page when it opens
  • We can suggest another provider now

Extended supports, in detail

When you need a bit more than everyday support

These four sit alongside the everyday supports above. They usually involve someone else as well, a hospital, a housing provider or an employer, and how they're funded depends on what your plan covers. So here's what each one actually looks like, and what to send us so the first reply is a yes or a no rather than another question.

    Home & living

    Accommodation support

A bright, comfortable living room in a shared home

Where you live shapes the rest of your week. J&J does not own or run housing. We work with housing providers across the Peninsula and list their homes, so you can see what is actually out there. That covers SDA and SIL, plus short stays under STA and MTA. Short stays are sometimes called respite. We'll explain what each of those means in plain English. Funding depends on what your plan includes.

    • Homes from the providers we work with, with photos
    • SDA, SIL, STA and MTA, explained plainly
    • Short stays and respite
    • Everyday supports and nursing once you're settled
    • Help to work out what your plan funds for this

What to send us. Tell us the suburb, who else is involved, and whether this is a plan review, a move, or something that has to happen soon.

    Clinical

    Registered nursing

A nurse providing care at home

Where the clinical side sits alongside everyday support. Our registered nurses assess what's needed before anything starts, then set up the care plan the support workers follow, with the nurse still across it. Available through an NDIS plan or privately.

    • An assessment by a registered nurse first
    • A care plan the support workers actually follow
    • Nursing and daily supports on one roster
    • We talk to your GP and specialists when you want us to
    • Funded through your plan, or privately

What to send us. Tell us what the clinical need is, how often it's needed, and who's looking after it now.

    Post-hospital

    Hospital discharge

A nurse supporting someone recovering at home after a hospital stay

Coming home is the part that gets rushed. Give us the discharge date and we work backwards from it: what the first week needs, who's coming, and what has to be sorted before you walk back in the door. Nursing sits on the same team as the support workers, so clinical care and daily help come off one roster.

    • Give us the date and we plan back from it
    • The first week set up before you get home
    • Registered nurses on the same team as your workers
    • We'll talk to the ward, the coordinator and the family
    • Short notice is normal, ask us anyway

What to send us. The discharge date, the suburb, and what the hospital has said you'll need at home.

    Work & purpose

    Employment pathways

    Building work-ready skills together

Work, or a step towards it. Resumes, applications and interview practice first, then the part most people need more help with: getting there, the first few weeks, and what to do when something at work isn't working. If what you need is a specialist employment program, we'll tell you and help you find one.

    • Resumes, applications and interview practice
    • Confidence before you start, and after
    • Getting to work and settling in
    • Volunteering or study as a first step
    • Working out what your plan funds for this

What to send us. Tell us what kind of work you're after, what you've tried already, and what gets in the way.

    Enquire about employment pathways

Beyond the NDIS

Private and in-home care, without an NDIS plan

Not everyone we support has an NDIS plan, and that's completely fine. Private in-home care is available to fund yourself, with the same local team you'd get through the NDIS. We'll give you a written quote before anything starts. One thing to know: we're not registered for Aged Care. If that's what you need, we'll point you somewhere that can help.

  • Personal care
  • Post-hospital care
  • Household assistance
  • Meal preparation
  • Medication support
  • Transport
  • Social support
  • Technology assistance
